English Language Proficiency required. DUTIES:
The RN Case Manager (RNCM) possesses knowledge of clinical documentation requirements, current standards of care, and compliance guidelines. Accountable for utilizing critical thinking skills and sound judgment to ensure positive outcomes. The RNCM will develop and coordinate seamless transitioning of patients throughout the inpatient and outpatient care process with a focus on discharge planning needs. Demonstrates knowledge and ability to apply principles of evidence-based practice. The RNCM will provide support at the unit level, the service level, and the organizational level. A key aspect of the role will be discharge planning assessment and implementation of home health services, supplies, and activities required to successful transition the patient to the appropriate non-acute care setting. Coordinator will serve as a ! representative on medical center and service committees. The RNCM will be working with Surgical service on surgical patients. Specific functions and responsibilities of a RN Case Manager include:

1. Provides initial and ongoing assessment of patients to identify needs issues, care goals and appropriate resources necessary for care management.

2. Provides leadership in application of the nursing process and identifies resources and critical factors for achieving desired outcomes for discharge, post hospitalization recovery and health maintenance/improvement.

3. Sets clinical care goals, short and long term, in collaboration with patient, provider(s), and family members.

4. Functions as a system coordinator for the plan of care; monitor progress through the expected treatment course and intervenes to facilitate achieving patient outcomes within anticipated time frames. Coordinates care and discharge planning with the patient's primary care ! provider and team.

5. Demonstrates knowledge of discharge plan! ning and understands the level of care to match the Veterans/patient needs with the most appropriate level of service.

6. Demonstrates knowledge of regulatory requirements of various agencies to include Center for Medicare and Medicaid Services (CMS), VHA, The Joint Commission (TJC) and insurance systems.

7. Utilization Review (UR) and Management: Demonstrates knowledge of UR process (InterQual) validated by inter-rater reliability reviews. Demonstrates knowledge of how to use Medical Center/Care line resources to provide the highest quality of care in the most cost-efficient settings.

Hours : Monday - Friday, 8:00AM - 4:30PM

2 days at the Rutland Community Based Outpatient Clinic and 3 days at the Bennington Community Based Outpatient Clinic. QUALIFICATIONS REQUIRED:

Citizen of the United States.

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredite! d by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia . In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Must be proficient in spoken and written English .

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of qual! ity with regard to the following four dimensions of nursing: Practice, ! Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research. Education/Experience Requirements:

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 year! s of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
