The Visiting Nurse Association is excited to announce the opening o! f a per diem position for a SASH nurse! Provide a variety of clinical services with the goal of enhancing, promoting and maintaining the health of residents living at senior housing facilities.
JOB OVERVIEW
- Facilitating safe and effective transitions for patients between off-site care facilities and home.
- Encouraging and promoting the importance of self-management of chronic health conditions.
- Conducting health and wellness assessments, health education, foot care and medication management.
- Current RN license with at least two years of experience in a clinical setting.
- A commitment to an aging in place philosophy.
- Must be able to work independently and possess strong organizational and time management skills and excellent verbal and written communication skills.
