As part of the Vermont Chronic Care Initiative, APS will be assisting with the Vermont Advanced Improvement Program which features key components of a statewide technology infrastructure to improve care coordination for members with chronic illness and further reduce avoidable costs of care. This infrastructure solution is based on innovative technology for care management, delivery of evidence-based interventions by Care Coordinators within the Department of Vermont Health Access (DVHA); and pharmacy analysis and prescriber feedback. Another focal point of the program includes assistance and training on the use of the technology and information products as well as collaborative support for provider and beneficiary interventions.

APS is looking for a RN Care Coordination Liaison (Clinical Specialist Liaison) to support this exciting program. Alongside the Vermont Chronic Care Initiative Field Direct! or, this key role facilitates alignment between APS Clinical P! ractice Liaisons and DVHA Care Coordination (CC) field staff to assure implementation of VCCI strategic efforts and achievement of clinical and financial goals. Serving as a consultant and subject matter expert, the Liaison provides a wide range of project management and clinical support services, supporting in the CC staff's success in their efforts to coordinate care and services for high-risk, high-cost members. This will include training and care supporting the CC staff in effectively using the APS CareConnection care management system, as well as developing and implementing process and quality improvements.

The ideal candidate will be technologically-savvy combined with strong clinical experience and the ability to train, support and mentor others. This position will be based both in the APS Williston office and the DVHA office and will require 50% travel throughout the state to work with the CC staff. Candidates outside of the Williston area can be considered! if they are willing to travel to the Williston DVHA office 2-3 days/week.

Essential Functions:

• Works with the DVHA Field Director and Case Managers to support the day-to-day operations of work assignments for DVHA Care Coordinator Team. Assures CCs have tools and support to provide care coordination to high-cost, high-risk members in accordance with Scope of Practice laws, and program outcomes for quality and cost.
• Assists Director in case load management for field staff based on clinical license, case load/case complexity, geographic coverage areas, case durations and reports on goal achievement.
• Develops a thorough understanding of the APS CareConnection care management application to become a subject matter expert.
• Provides in-depth program and ad hoc training on CareConnection (including the Percolator, assessments and provider-facing reporting tools) workflows, and documentation to CC staff and occasionally provi! der staff.
• Gains feedback from end users, and DVHA stakeholder! s on use of tools, technology and workflows.
• Assists with the development and on-going refinement of tools, technology, workflows, training, and allocation of work among APS and DVHA team members.
• Secures and assists in interpretation of various analytical reports with DVHA Field Director to inform course correction locally as indicated.

Education:

• Bachelor's degree in Nursing required.
• Active and unrestricted VT RN licensure required
• Certification in Case Management and/or Project Management preferred.

Qualifications:

• Minimum 3-5 years experience in clinical care coordination service setting, with previous experience in care coordination and/or case management.
• Experience in public health, insurance and/or other population based health care management environment preferred. Knowledge of public health and/or population based health care analytics and management highly ! desirable.
• Previous experience training and supporting all levels of clinical staff, including the use of electronic care management tools required.
• Previous team leadership experience preferred.
• Experience and comfort level working with behavioral health related issues preferred.
• Public sector experience or work with vulnerably, medically fragile patients preferred.
• Knowledge of Vermont Healthcare Reform, Vermont community and clinical resources, and Medicaid benefit plan.
• Strong technical competence with computerized systems (MS Office, EMR), including the ability to learn new technology quickly.
• Knowledge and experience with adult learning techniques. Comfortable facilitating and presenting to small and large groups.
• Strong clinical skills including knowledge of evidence based guidelines.
• Strong written and verbal communication skills.
• Strong coaching and mentoring skills.
