BASIC FUNCTION
To assist in implementing a medically-directed plan of care for patients in their homes; to assist patients! and their families with a variety of home management tasks and personal care activities.
CHARACTERISTIC DUTIES & RESPONSIBILITIES
Ø Includes basic nursing assistant and restorative duties for which the nursing assistant is prepared by education and supervised practice such as wound care, assistance with self-administered medications, assistance with therapeutic experiences, help with assistive devices, simple technical procedures.
Ø Assist in personal care of patients including ambulation, use of wheelchair/walker/cane and other adaptive equipment, transfer activities, range of motion, bathing, shampooing, hair care, oral hygiene, nail care, shaving, skin care, dressing, toileting/bowel programs/ostomy and catheter care, meal preparation, special diets, and other activities of daily living.
Ø On a limited basis, perform general housekeeping tasks such as:
- everyday housecleaning tasks: make beds; vacuum, sweep and mop floo! rs, wash dishes, and clean up kitchen areas, clean bathrooms, ! pick up and neaten living areas;
- laundry: wash, dry, fold, iron, put away clothing, bed and table linens;
- Assist in maintaining home environment, identify hazards and concerns.
Ø Confer regularly with primary Nurse and/or PT regarding patients' status and treatment plan.
Ø Document patient condition, service provided according to agency standards.
Ø Travel to and from patient's homes and VNA office.
Ø Other duties as requested by supervisor.
SUPERVISION RECEIVED
Close to moderate supervision (following Agency, State and Federal guidelines) is received from Registered Nurse or Physical Therapist.
QUALIFICATIONS
Basic education (high school), with p revious experience in nursing home or similar facility desired, or a combination of education and experie! nce from which comparable knowledge and skills are acquired.
Ø Vermont State Board of Nursing Licensed Nursing Assistant.
Ø Knowledge of physical, emotional, developmental needs of and ways to work with population served by home health agency.
Ø Ability to communicate and interact effectively with individuals and groups.
Ø Ability to identify emergency situations and act appropriately to provide assistance.
Ø Ability to work independently in private homes under the supervision of a licensed RN, PT.
Ø Ability to travel extensively in order to conduct home visits.
Ø Physical ability to perform all personal care activities and other essential job duties per current agency standards (Refer to LNA Physical Demands form).
Ø Ability to complete additional training appropriate to the job prior to assignment.
Ø Possession of current driver's license with a relatively clean driving history and t! he use of a dependable registered and insured car.
Ø Automobi! le Bodily Injury Liability insurance with coverage amounts of $100,000 per person, $300,000 per accident.
This position is 30 hours per week and is benefit eligible.
