As a member of an interdisciplinary team, which includes physicians, nurses, service coordinators, PT/OT, the Social Worker II is responsible f! or assessment and intervention with any patient or family along the health care continuum where psychosocial and/or high risk factors are a part of or interfere with the patient's treatment, health outcome and discharge plan. The Social Worker II utilizes advocacy skills, intervenes therapeutically supportively, provides resource and referral information, offers financial guidance and assistance, and assists with discharge and post discharge planning. The Social Worker II serves as a liaison to health and social agencies in the community.

This position will serve patients and their families in our Berlin Dialysis Unit. Please note that this is a part-time position. The employee works for 16 hours one week and 24 hours the next week. The typical schedule is 2 days one week; 3 days the next. The hours are generally 8:00 a.m â€" 4:30 p.m. This role could be combined with other part-time roles in the department.

The ideal candidate will have strong clinical, i! nterpersonal, interdisciplinary and computer skills. Additiona! lly, the ability to work independently is also important.

Education:
MSW from an accredited school of social work. Licensure through the State of Vermont is encouraged.

Experience:
Experience working in a human service organization, a health care setting, or a managed care system is required.
