We currently have a client that is looking for a Social
Worker.
Responsibilities:
Social Worker prepares social histories, assesses
patientâ s needs, provides social casework to patients
and families having difficulty in social functioning
primarily as it affects their health. Refers and aids in
the referral process to social agencies, and generally
serves as a social worker for the Agency .
Provides social casework to individuals and families
receiving nursing or other services from the agency in
accordance with the physicianâ s plan of care
Assists the physician and other health care providers in
understanding significant social and emotional factors
related to the patientâ s health problems in order to
develop and update the plan of care as indicated
Provides and documents services in accordance with state,
Medicare, Medical, and other applicable regulations and
standards and third party payer guidelines
Uses
appropriate Infections Control procedures and safety
measures
Assesses patient and family and
prepares social histories to augment existing service! , or
as a guide in determining or changing level of servic! e
Assesses, when appropriate, a
patient/familyâ s financial situation taking into
consideration patientâ s prognosis and medical needs,
referring to an agency for financial assistance when
indicated, interpreting the medical situation to the
referring agency and generally facilitating the process of
rehabilitation
Assists patient and family with
personal and environmental difficulties that predispose
toward illness or interfere with obtaining maximum benefits
from medical care or cope with terminal illness
Refers patients and families to community agencies to
utilize available resources to rehabilitate the patient or
assist the patient to live with the disability or terminal
illness
Works with patients, families and the
health care team effectively
Observes, records
and reports social and emotional changes appropriately and
at least every sixty days
Com! pletes clear
concise and accurate clinical notes, which document skilled
services appropriately, and submits all requested
documentations on a timely basis
Participates in
case management, orientation, case conferences, staff
meetings, care planning, discharge planning and in-service
programs and acts as a consultant to other agency
personnel.
Requirements:
Prefer Masterâ s degree from a CSWE Accredited
School of Social Work.
Experience in a hospital setting.
Job Requirements
Vermont License is required. MSW/BSW; as well as Discharge
planning experience.
