Case Manager II - Per Diem - RN and/or MSW

Location: Burlington Vermont

Description: Fletcher Allen Health Care is looking of Case Manager II - Per Diem - RN and/or MSW right now, this occupation will be placed in Vermont. Coordinates the care and services of selected patient populations across the continuum of illness; promotes effective utilization and mo! nitoring of health care resources; and assumes a leadership role with the interdisciplinary team to achieve optimal patient-centered, clinical and resource outcomes. Proactively identifies and intervenes to address barriers to treatment, health outcomes and discharge planning including biopsychosocial and other high risk factors. Serves as an important link between the patient, the healthcare team, the payer and the community.

The candidate will need to have the interest and flexibility to provide coverage for a variety of case manager assignments.

Education:
The position requires a level of education or the level of knowledge usually acquired to become a Registered Nurse (RN) in the State of VT, with Bachelor's of Science in nursing preferred or related collegiate courses, or a Master's in Social Work (MSW) from an accredited school of social work.

Experience:
RN: Minimum of (3) years of clinical experience in a related fiel! d. SW: Minimum of (3) years experience in a clinical setting, ! including (2) years in a medical setting, preferably a hospital. Both RN & SW: (2) years case management experience or a similar role preferred.

The ideal candidate will have discharge planning and/or utilization review experience as well as a strong clinical background working in a hospital and/or home health care.
