APS Healthcare has an exciting opportunity for an RN Care Manager in our Williston, VT office. The Vermont Chronic Care Initiative features key components of a statewide technology infrastructure to improve care coordination for Vermonters with chronic illness, further reduce avoidable costs of care and provide patient education and self-management support. APS Healthcare utilizes innovative technology for care management, including the delivery of evidence-based interventions, pharmacy analysis and prescriber feedback.

The RN Care Manager develops and maintains strong, positive relationships with healthcare providers (acute, ambulatory, medical and behavioral) to ensure high-risk, high-cost members have open access and appropriate care. Working side by side with the DVHA staff, ensures an open flow of communication between providers, community-based and tel! ephonic care coordination efforts regarding members. The RN wi! ll also provide telephonic follow up to members as appropriate. This position is based in our Williston office and will require some travel to areas of the state needing provider outreach.

Responsibilities:

  • Contacts and visits community providers (hospitals, ambulatory providers of medical and behavioral healthcare) who serve high-risk, high-cost Medicaid members. Reinforces purpose and value of outreach initiative.
  • Offers providers ‘staff population management tools derived from claims to identify patients with gaps in their care.
  • Provides consultation to providers on strategies to “close” gaps in care and reduce fragmentation of care, including leveraging care coordination and telephonic staff to impact appointment, medication and treatment adherence.
  • Supports provider quality improvement initiatives/interventions, including outreach and education programs.
  • Closely collaborates with Community Health in! itiatives and programs to ensure that APS is closely aligned with those initiatives with both providers and members.
  • Maintains a case load (typically 50 or less members) of telephonically care managed high risk members.
Education:
  • Active and unrestricted VT RN licensure required
  • Associate’s degree or Diploma in Nursing required.
  • Bachelor’s degree in Nursing or other Health Related Discipline preferred.
Qualifications:
  • Minimum 3-5 years experience in clinical care coordination service setting, with previous experience in care coordination, case management and/or disease management preferred.
  • Previous experience in quality improvement, office or outpatient nursing helpful.
  • Experience and comfort level working and creating successful partnerships with medical providers.
  • Experience and comfort level working with behavioral health related issues preferred.

  • Knowledge of Medicare/Medicaid needs of medically and socially fra! gile patients.
  • Strong technical competence with computerized systems (MS Office, EMR), including the ability to learn new technology quickly.
  • Knowledge of adult learning and behavior change principles preferred.
  • Ability to train varying levels of professional employees of clinical practices.
  • Strong written and verbal communication skills.
  • Strong decision making and problem solving skills.
