The Case Manager promotes patient's health, maintenance and restoration to provide qualit! y, cost-effective outcomes; initiates a discharge plan to meet immediate post hospital needs; visualizes long term needs to optimize health maintenance and prevent re-admission; educates patients, family and staff; collects, analyzes, and interprets various data to evaluate current opportunities to expand current clinical knowledge, as well as cost containment strategies.
Past clinical case management experience preferred. Two years of medical/surgical or related experience required.
Graduate from an accredited school of nursing. BSN required, BLS, Case Manager Certification within 2 years of hire.
