Clinical Auditor/Coder

Location: Vermont

To support clinicians in producing accurate, high quality documentation by assis! ting with the complexities of coding, providing ongoing education on quality documentation and providing the appropriate guidance to clinicians on utilization of services based on the plan of care.

CHARACTERISTIC DUTIES & RESPONSIBILITIES
Primary
 Select and correctly sequence the appropriate diagnoses and their corresponding ICD-9 codes for each patient referral/episode:

  • Review documentation;
  • Coordinate with the I&R Staff to obtain referral information;
  • Contact referral source or physician for any necessary clarification;
  • Educate, troubleshoot and answer questions from clinicians regarding selection and sequencing of diagnostic codes.
 Audit clinical documentation to identify and correct errors, omission and/or inconsistencies in OASIS assessment, Plan of Care and other related documents:
  • Review documentation;
  • Document and provide reports on results of audits;
  • Unlock co! mputerized documents for correction and completion;
  • E! ducate, troubleshoot and answer clinician questions regarding their OASIS/assessment documentation.
 Other duties as requested by supervisor.

SUPERVISION RECEIVED
Minimal supervision is received from the Manager of the Clinical Outcomes Team.

QUALIFICATIONS
RN licensed in Vermont, plus five years of clinical nursing experience and 3-5 years home health experience, or a combination of education and experience from which comparable knowledge and skills are acquired.
 BSN or equivalent desirable.
 Coding Certification and experience with ICD-9 coding.
 Computer proficiency.
 Strong attention to detail and critical thinking skills.
 Ability to converse comfortably and clearly with a variety of people and to deal with callers and staff in a courteous and efficient manner.
